Missionview Church will not give medication to any student attending our services, activities, or events unless authorized by your acknowledgement below.
With full knowledge of emergencies, dangers and risks related to the administration of such medication by MissionView Church employees, officers, or agents, we hereby waive all claims which might arise from said administration of such medication to said minor child and the results thereof. We agree to indemnify and hold harmless MissionView Church employees, officers, or agents from any and all liability relative to the administration of such medication
I understand that MissionView Church is offering my child the opportunity to participate in programs and various recreational activities. I fully understand that acknowledge that (a) risks and dangers may exist in my child's participation in, use of equipment associated with, and transportation to and from activities and (b) my child's participation in, use of equipment associated with, and transportation to and from activities may result in injury or illness including, but not limited to, bodily injury, disease, death, or other ailments that could cause serious disability; (c) these risks and dangers may be caused by the negligence of volunteers, members, employees, officers, or agents of MissionView, the negligence of the participants, the negligence of others, accidents, or other causes; and (d) these risks and dangers may arise from foreseeable or unforeseeable causes, hazards, and dangers. My child is in good health and I understand that physical exertion may be required to participate in the activities. My child has no known physical disabilities or health problems that will prevent any risk to his or her participation. By signing (typing my name) below, I grant permission for my child to participate in the activities, to use equipment associated with the activities, and be transported to and from activities, and I assume all the risks associated with the activities.
In consideration of MissionView Church offering and organizing activities, I agree as follows; I on behalf of myself, my child, my spouse, my personal representatives, relatives, heirs, and assigns, hereby voluntarily agree to release, waive, forever discharge, hold harmless, defend, and indemnify MissionView Church and it's volunteers, members, agents, officers, elders, and employees from any and all claims, actions, causes of action, damages, expense or losses for bodily injury, property damage, wrongful death, loss of services and consortium, or any other loss or damage of any kind whatsoever which may arise out of my child participation in the activities, the use of equipment associated with the activities, and transportation to and from the activities. I especially understand and agree that I am assuming all risks and dangers and all responsibility for any claims, whether caused by or negligence of MissionView, the negligence of the participants, the negligence of others, accidents, or other causes, and that I am releasing, discharging, and waiving any claims that I may have, presently or in the future, for the negligent acts or other conduct by MissionView.
I have read the above parent/guardian permission form and waiver and release and, by signing it, agree it is my intention to permit my child to be transported to and from the activities and to engage in the activities, and to exempt and relieve MissionView from any and all claims that may arise from my child participation in the activities offered as part of MissionView Church.
